Agent Operational Issue

Custom object AgentOperationalIssue__c. A failure that needs a person: a lost start, a failed approval relay or notice, a failed worker, or a recovery step that could not finish. Core records one open issue per kind of failure and subject, counts repeats, and says what to do next. It holds no message content.

Closed At	ClosedAt__c	DateTime	When the issue was resolved or dismissed. Record retention deletes it 30 days later.
Closed By	ClosedBy__c	Lookup	User who dismissed the issue. Blank when Core resolved it.
Code	Code__c	Text	The kind of failure, such as FLOW_START_LOST or WORKER_FAILED.
Command ID	CommandId__c	Text	The Agent Command the failure concerns, when there is one.
Detail	Detail__c	Text	A short, content-free detail: an exception type, a reason code, or which agent and record a lost start was for.
First Seen At	FirstSeenAt__c	DateTime	When this failure was first signalled.
Last Seen At	LastSeenAt__c	DateTime	When this failure was last signalled.
Next Step	NextStep__c	Text	What an admin should do to fix this kind of failure.
Occurrences	OccurrenceCount__c	Number	How many times this failure was signalled while the issue was open.
Open Key	OpenKey__c	Text	Code and subject while the issue is open; blank once it is closed, so a later failure opens a new issue.
Run ID	RunId__c	Text	The Agent Run the failure concerns, when there is one. Text, so record retention can delete the run first.
Status	Status__c	Picklist	Open until Core sees the subject recover (Resolved) or an operator dismisses it (Dismissed). Values: Open, Resolved, Dismissed.
Subject ID	SubjectId__c	Text	The approval or approval work item the failure concerns, when there is one.
Subject Key	SubjectKey__c	Text	What the failure is about: an approval, work item, command, run, or a hash of the lost start.
